Mac - I record here from my Garmin, it logs every set with a weight and exercise type which I always have to refine but it will do until I start a strength programme again, that is based of a spreadsheet I have with macros in tow work out percentages of my 1rm for each lift to know what weight to lift as the rep range changes on a weekly basis.
Happy - training to failure has its place, but as a general rule, the best benefits seem to be gained with less negatives by leaving at least a couple of reps in reserve at the end of the set. Moreso as things get heavier and with compound lifts though, not as important for lighter, isolation work.
Anybody that is worried about safety with bigger weights, maybe see if you can find a cheap wall mounted or free standing half rack with safetys on to catch the bar should you fail. That and/or look up how to bail out of the lifts you're worried about. Bench is painful but generally not dangerous, squats can be but not too bad if you have clear floor behind you and know what you need to do with your arms.

new squat PB this morning 4 x 5 x 140Kg, felt like my form was OK, didn't do the final set of five, because the complainy muscle on the inside of my left thigh started complaining

Tried the deadlift again today, went heavier, and the grip strength was the biggest struggle. Maybe I shouldn't be doing sets of 5, maybe I need straps or hooks

I went to the gym on Monday, for the first time in a very long time, thank goodness for the pink plastic weights

Oh dear Oscar, hope it soon fades.
I was mildly sore and its gone now so back to it again, but will be another very easy one, and for a week or two year building up very slowly
