There is an important difference between inattentive driving and inconsiderate (and dangerous) driving.
My experience suggest most of my close encounters are with drivers who - misjudge the situation such as room, speed of oncoming vehicles etc - wilful actions irrespective of the conditions which they should be well aware of and not giving a flying one whether they or anyone else is in danger
- impairment due to drink, drugs, health conditions, lack of sleep
All of the above, in my opinion and experience , wearing hiviz, many lights make no difference whatsoever. Sadly the lights / bright colours are there to as victim blaming avoidance should one have such an incident. A driver would be hard pressed to get away with saying they didn’t see me. But it had not prevented any of the closest calls I’ve had. Thankfully, I have managed to anticipate and take evasive action in these cases so far. I treat every driver like they’re an idiot!

I've just tried to plug my rear tyre - only to make the problem far worse.
Is there an idiot-proof solution, or should I just give up and go back to tubes (in the certain knowledge that I'll get a puncture every time I ride)

I guess it's a combination of the amount of crud on the roads, and being fat. I used to get one pretty much every time I rode when on tubes - an experience I am not keen to repeat. I went tubeless in May, initially had a few issues with the puncture-ridden ridden tyres from my tubed days, so replaced them - and since then have been trouble free until today. This one was caused by a small shard of glass (which I removed) - but trying to plug it has just made matters worse - all I've done it turn a slow puncture into a fast one.

I've possibly asked this before, but what tyres are you using and at what pressure? In addition, have you checked it's not an issue with the wheel?

It's not an issue with the wheels (Hunt 4 Season Superdura) or tyres (Schwalbe G-One 35mm, 55psi front/60psi rear) - I've had ~6 months and over 1,000 miles puncture free since ditching the tubes. I know exactly what caused this puncture - it was a small shard of glass. I tried to plug it, and now I have a far worse puncture.

In my experience once the hole is big enough to not seal then it’s time for a new tyre.
Has your sealant dried up or needs topping up? You still need some sealant with a plug. I’ve got the dynaplugs that have a small metal point and you just push them through, it’s been enough to get me home when I’ve had a larger hole.

We had a nightmare on the club ride on Saturday. Clubmate had picked up her winter bike without checking it over or riding it and forgotten to put her saddle pack on. So we had to donate tubes and air. First tube the valve was stuck. Second tube worked but the tyre blew off the rim when inflating. Then the valve cam right out in the inflation device. Then the tube wouldn’t hold any air at all, probably damaged from faffing around.
She ended up calling for a lift home, while the rest of us ended up trying to thaw out in the cafe.
Lesson, check your spares occasionally, and never go out without them.
