I think I have only done a course with steps at Wycombe Rye, wooden steps with awkward sizes.

One Thousand Odd Spiders
Cross Flatts 'had' steps, but they resurfaced a path that bypasses them, and the parkrun route avoids them now..

Not sure the big puddle at Milton counts as a puddle of doom (no more than 50cm deep at its worst) but it does sometimes have its own puddle marshal, directing runners to go through the puddle, and not slow down for it or try to go round the edge. It's on a fairly congested stretch and someone stopping to avoid could lead to hundreds of people behind crashing into each other, keystone cops style.
