[My suspicion is one main ring circuit at first floor level, with pipes to each radiator, either down for the ground floor ones or 'up' for the first floor ones. All put in after the main build, and if CH piping then probably electrics as well, and then plaster board hammered up to hide everything]

Our central heating pipes are buried in the concrete slab downstairs, the house is 45 or so years old. Those pipes are one of the reasons I’d like to move - before the fall apart or fur up completely.

Our water pipes (for the underfloor hearing) are in the screed, so yes a big job if there is an issue.
This is still i believe the recommended approach, ours was only done 6 years ago
