So you have any barrel adjuster working? Dependent upon the groupset, there may be an adjuster on the mech, and one near the bars. Otherwise you can get inline adjusters that go midway along the cable outer.

You may be able to get away without needing to rethread at the shifter, and just change the outer cable somewhere along the length.

The barrel adjuster near the derailleur is working fine. It's the one near the top of the handlebars that has broken:

Beginning to think it has snapped off completely as I think there's a bit still inside the bike section...

So in other words, I think I need to get the other section of the barrel adjuster out from the cable housing first right? I have some wd-40 like stuff...

Ah a set of bolt extractors (only £3.52), then a new barrel adjuster sounds like the best bet I'm thinking now.

The threaded bit may have broken but alternatively is it just fully withdrawn so far that it no longer engages in the other part? The design isn't entirely clear to me. It looks a bit suspiciously clean for a break.

I agree with JDA. I'd check if there is actually still left in the lug. I'm not sure if you know how they work, but if not, they have two coaxial parts that use a thread to shift along each other. If you've pulled the inner entirely into the outer, it may just need the two parts turning to show the inner again and allow it to re-engage with the lug.

Ok yes possible hard to tell. Ok two more pics, first the cable housing:

And second the barrel itself:

The other barrel I have is quite long, this one has 1 mm most of thread showing, and just doesn't engage, even without the spring. I can't see that it can have slipped inside the body of the barrel, because if I flip it around there is nothing there for it to slipped into - it's empty space. So I'm guessing you mean the cable housing? Or that it has snapped off?

Jam 1 pincer of a needle nose plier in there, you'll be able to wedge it in enough that you can then twist the broken bit out. Any extractor set will kill the threads in there I reckon.

Yeah ok that does now look like a neat break to me. I assume it will be hard to get the broken bit out, the reason it broke is presumably that it corroded in place!

Assuming it’s unsalvageable you should be able to get an in-line barrel adjuster that fits along the cable housing independently of any stop on the frame. I’ve got those on one of my bikes, it’s what it came with. You just cut the cable outer and install it between the two cut ends.
