I reset as was in div 12 (ish) and now sat mid table 24. Am stuffed for while as being very average and lots of peeps joining and rejoining. Might couple months for Utmost to settle down

If you count the number of ringers above you, and divide by three, that's roughly how many weeks it'll be before you see some progress.

There are a few divisions with only 17 (or even 16) runners. Will those gaps just filter down gradually or will there be bonus promotions for those outside the top 3?

As things stand, there are never more than three promotions, and up to three relegations. So the gaps will move down slowly.
We have four divisions with a full complement of 20 players; seven divisions with 19 players; six with 18; six with 17; one with 16; and the current bottom division (div 25) with 11 players.
Overall, we have 450 players, which is up on (I think) about 400 before the new badge news

Given my current health (or lack thereof), the only way I'm likely to see a promotion is from people above me dropping down. If my counting is right, there are 36 gaps above me, which would take me up a couple of divisions - so I just have to hope that the taking up of the slack is faster than the rise of the speedsters who've taken a dive to rejoin the back of the pack. (And, of course, I'll lose more than half of my points in August, when my last half & 10mi drop out of contention)
How exciting.
