If you want people to volunteer unnecessary information you should make the collection form as simple and self explanatory as possible - requiring them to open a separate, password protected, explanation page is too high an entry barrier for many, including me.
And sending sarccy impersonal responses when someone *has* bothered to return your badly designed form isn't going to incline anyone towards going back and redoing it.

(I may have said a condensed version of this, and probably shouldn't've, but oh well)

I didn't wuss out of going to rugby training tonight on the flimsy premise that the ground would be slippery after weeks of being baked hard and rained on all day, and training in the continuing rain will make it more likely noses will be running and we'll be inadvertently sharing things we ought not to be.
It's not at all that I just didn't fancy the mild discomfort of being wet and cold.
