And, when I've got to the end of 3rd/4th size (I'm making 4th size), I've measured and got this:
It's WAY too long already. Wtf have I done wrong? How do I remedy it?

It looks as if you should have done the first bit (the 4th row), then ignore the every following 22nd row and use the every 8th row, as specified for the 3rd and 4th sizes only.

I am trying to count how many rows for the total increases - 29?
Something seems to have changed halfway down the moss stitch bit.

Yeah, I did a normal moss stitch row rather than Irish moss.
Right. I am going to try Garfield's suggestion... that seems odd though, given the instruction above for every 26th row... wash.

Yeah, it's a pain when that happens! I've had my share of miscounts in the past on slightly larger scale projects! Aran cable sweaters and one really ambitious sweater I tried making in my early 20s, which I gave up in exasperation! Mohair and cotton, it would have been beautiful if it had turned out but ended up an exercise in futility!!

4th row, then 6 times every 8 rows = 48 + 4 = 52 rows should see you with your total required increases to get the right number of stitches on the needle.
