Starts at the Coliseum which is pretty awesome, fairly crowded to get to the start but the roads are pretty wide so you can get into your stride farily early on. First few miles are a bit dull as you head out of the city but then you head back into it again and some great scenery. We didn't go that close to the Vatican this year cos it was right around the time of the enthronement of the new pope so they restricted access to that bit, but rest of it was good. Surprisingly it's pretty flat most of the way round, bit of a climb at somewhere around mile 17 I think but not to severe. Climb up to the finish which is at the Coliseum again but you're that close to the finish you probably don't care. All in all great city, great event. Hope you enjoy it. Am going back to Italy for my second one but Venice this time.
