I'll answer for 57.5 and say McCullum is no2 in the 2nd batting inns list 
It's a famous inns for other reasons.
All the top ones are in the 3rd inns of the match but the highest 4th inns total is very impressive.

Ashton Agar. 98 ( I think). On debut?
It was around the time I remember England being incapable of dismissing the tail. Tino Best (?) got 90ish as well around the same time.

Which two* male players have scored 5000 Test and ODI runs, taken 100 Test and ODI wickets, and taken 100 Test and ODI catches?

Actually, disproving my previous point I think Carl Hooper is the other one (and yes it saddens me that retiring in 2003 no longer seems to count as the modern era).

1) It was on debut
2) It was part of a losing effort
3) Given they got got gubbed the Aussies probably weren’t cheating at this point
3 is probably the most notable.....
