It may be a niche Brummie thing. I've never come across it being called a twinkle anywhere else.

So a little girl in the USA goes to with her dad to the barbers. Her dad gets into the barbers chair, and the little girl steps up alongside the chair too. The barber says to he "Step back a bit honey, you're gonna get hairs on your Twinkie." To which she replies "Yes, and I'm gonna get boobs too."
