Stress fracture

Hi, I have a knee problem, I have been got the Physio who is saying its irritation under the kneecap. He isn't probably right but with rest it is not improving which has me a bit paranoid about a stress fracture. I don't have swelling I can't find a painful area to prod It doesn't hurt when I walk/ cross trainer I can hop fine on one leg It hurts when I run and cycle then will ease off after a few minutes or hours. The pain is sometimes on the inside sometimes underneath sometimes just general I am sure Physio is right but it has been going on for 4 weeks now. Initially I did some short runs then I rested tried again rested etc. Just had 5 days rest and managed 2 minutes on the treadmill before stopping then had 3 more days and did 3 mins. The pain is not really bad but I don't want to be making it worse so stopped I am continuing to rest but would like to rule out a stress fracture as it is stressing me out |

Can you get stress fracture of the knee without knocking it? I'm asking as I don't know. You can ask for a referral to an ortho from your docs to be sure, but unless you go private you will be in for a long wait. You'll need a bone scan on the affected area usually (not a lot tends to show on xrays). Its pretty uncomfortable procedure. If they suspect anything ortho you'll be treated at the fracture clinic most likely. Usually recipe for healing is 6 weeks rest. |

Where is it sore? Is it the actual joint or your thigh? I'm not a sports physician but would think that a stress fracture more likely in either your femur or tibia than your patella given the patella doesn't bear weight? MRI would look at the knee in more detail. You'd probably need to be referred to ortho for them to request imaging unless you have a sports injury clinic. Can you get any of the physios through running with the England Ultra team thing to look at it? |

L it changes where it hurts sometimes just inner knee sometimes under but always the patella area Rest = nothing that makes it hurt so I am at the gym as Physio said that was ok if not hurting |

Pretty rare, but not impossible. I think that it is more common if you have had previous non stress fractures there. MRI is the gold standard for diagnosis, complete rest is the only treatment. Much more likely to be something like the physio says, but I can see why you are getting frustrated after 4/52 of waiting. I would probably go to complete rest and see if that helps. |

Or get your GP to refer you to a sports consultant to get an MRI?

the thing I have read is that MRI often dont show up SF have been reading that ultrasound can be used Have booked to see a diff physio, even if I just get confirmation that mine is correct it will be something I suppose. |

MRI is the gold standard, but I think that sciintigraphy (the bone scan) is still also used.Studies of other tests compare their effectiveness to these two. USS- I am not so sure, but it wont be able to demonstrate a lesion on the joint surface anyway as it would be behind other structures. It would possibly be more useful in problems in the shaft of a long bone. |
