For now we can trial an increase in medication dosage and see if that helps. No lumbar puncture or admission for now. A and E if any change. Blood gases on Thursday or Friday to check his meds aren't screwing his innards up too much. (He needs to be out of whack enough for the meds to affect his condition but not so out of whack he's puffing and panting and his kidneys fall out).
